(1220) Characterization of Asthma and COPD Ecareplan Data from a Pennsylvania Medicaid Payer Program Among a Clinically-integrated Network of Community Pharmacies

Objectives: The objective of this study is to characterize pharmacist electronic care plan (eCare Plan) data from a clinically integrated network (CIN) of community pharmacies for a regional Medicaid Managed Care Organization (MCO) asthma/COPD payer program. The Pennsylvania Pharmacists Care Network (PPCN) is a CIN of over 200 community pharmacies that provide enhanced patient care services. In 2021, PPCN established a contract with a regional health plan to enable pharmacists to manage eligible patients with asthma and COPD. Interventions in the contract include medication management, adherence assessments, patient education, and disease state clinical assessments. Documentation of interventions for each patient encounter via eCare Plans is required for payment for services by the health plan.

Methods: This is a retrospective study of program eCare plan data from April 2021 to May 2023. Deidentified data to be analyzed includes proportion of eligible patients at each pharmacy, number of patient encounters, encounter type (i.e. initial or follow-up visit), encounter method (i.e. in-person, telephonic), and interventions provided. Data will be analyzed with SPSS and descriptive statistics will be used to characterize the data. This project was reviewed by the University's IRB and was designated as not human subjects research.

Results: There were 23,637 eCarePlan submitted by 109 PPCN pharmacies included in the analysis. Data analysis is ongoing.

Conclusions: This data describes the care that community pharmacists provided for patients with asthma and COPD for 33 months as part of a Medicaid MCO program. Characterization of pharmacist eCare plan data will illustrate the value of community pharmacist-provided care for this Medicaid payer program. This research will support efforts in advocating for the role of community pharmacists in managing patients with chronic illnesses. This report will also inform future partnerships between pharmacy CINs and payers.
