Managing people with diabetes at transitions of care (TOC) is complex.  From fluctuating blood sugars to new injectable medications to insurance coverage issues and more, people with diabetes need the support of a pharmacist to ensure effective transition.  Attend this case-based discussion to practice navigating through the complexity of TOC in people with diabetes.
