Heather A. Johnson, PharmD, BCACP is currently an Assistant Professor of Clinical Pharmacy and Family Medicine at West Virginia University. She is a graduate of the University of Pittsburgh School of Pharmacy and completed her residency at The Ohio State University College of Pharmacy and the general internal medicine clinics. Heather’s primary practice site is in family medicine where she participates in interdisciplinary clinics for diabetes, weight management, women's health, COPD, geriatrics, and polypharmacy. She is also the program coordinator for the WVU School of Pharmacy PGY-2 in Ambulatory Care. Dr. Johnson is the Diabetes Managment SIG Coordinator-Elect and serves on the PCMH/ACO SIG Education & Resrouces Committee.
