Nutrition is a critical part of health, and better nutrition leads to better outcomes. Attend this session to learn about a unique model out of San Francisco Department of Public Health where primary care clinical pharmacists participate in Food Pharmacies and provide their patients with medically supported groceries and connectivity to healthcare services.
