(1026) Pharmacist-led Anti-hyperlipidemic Screening at an Outpatient Family Practice Clinic

Objectives: Cardiovascular disease is the leading cause of morbidity and mortality in the United States. The link between lower levels of low-density lipoprotein (LDL) cholesterol and reductions in atherosclerotic cardiovascular disease (ASCVD) has been well established. The 2019 ACC/AHA guideline on the Primary Prevention of Cardiovascular Disease has set a standard for proper strategies for prevention and management of CVD. Statins, PCSK9 inhibitors and Ezetimibe are therapies utilized in management of LDL. Statin therapy is first-line treatment for primary prevention of ASCVD in patients with elevated LDL cholesterol levels (≥190 mg/dL), those with diabetes mellitus, who are 40 to 75 years of age, and those determined to be at sufficient ASCVD risk. PCSK9 inhibitors and ezetimibe have a place in therapy for prevention and treatment in patients who are unable to tolerate statin therapy, add on to statin therapy for additional LDL lowering and or in patients with homozygous familial hypercholesterolemia (HoFH). Pharmacist–led interventions of statin initiation therapies have been conducted in the community pharmacy setting and resulted in successful implementation of guideline directed statin therapy in patients.To the best of our knowledge, outcomes related to a pharmacist-led anti-hyperlipidemic intervention in a free-standing primary care practice site has not been published, making this a first of its kind. The objective of this study is to demonstrate effectiveness of pharmacist-led initiative of anti-hyperlipidemic therapy in a family medicine practice site.

Methods: This is an observational study conducted at an outpatient ambulatory care clinic composed of a multidisciplinary team of physicians, physician assistants, nurse practitioners and pharmacists who see approximately 26,000 patients per year. The physician groups are composed of internal medicine and family practice. Patients will be included in study if they have a pharmacist scheduled appointments during the study period of Oct 1, 2023 to February 28, 2023. Patients will be excluded if they are under the age of 18. The 2019 ACC/AHA Guideline on the Primary Prevention of Cardiovascular Disease will be used to determine if patients’ are on appropriate cholesterol management therapies. The primary endpoint is to determine the percentage of patients who qualify for a guideline recommended intervention. Secondary end points of this study will be the acceptance rate of providers of patients who qualify for an intervention as well as a description of the types of recommendations made (e.g. dose optimization of current drugs, addition of new medications, etc) and the change of cholesterol from baseline for patient’s who had a successful intervention. Data will be summarized as a descriptive analysis. This study was approved by the Institutional Review Board.
