(1065) Evaluation of the Impact of Community Pharmacist Nutrition Focused Educational Intervention to Increase Patient Adherence to Diabetes Medications and Decrease Self-reported Adverse Effects

Objectives: The primary objective of this study is to evaluate community pharmacists from our institutions’ impact on increasing adherence to medications indicated for type 2 diabetes by enhancing patient understanding and awareness of dietary needs. The secondary focus is the assessment of reduced frequency of patient reported adverse effects. Diabetes is a disease that affects millions of Americans every year. Community based pharmacies can offer expanded access to medication services. Community pharmacists therefore can help increase adherence to guideline driven pharmacotherapy interventions that can result in improved patient health outcomes and reduced financial burden on the health care system.

Methods: The study is a quantitative single arm pre-post intervention analysis. The cohort of interest is patients able to be surveyed and provided targeted counseling on their medication indicated for type 2 diabetes between November 2023 and March 2024. Patients must meet the following criteria to be deemed eligible: be presenting to or receiving communication from one of our pharmacies, be over the age of 18, and be on a medication indicated for type 2 diabetes. Patients will be excluded from the analysis if they are only receiving insulin for treatment of their diabetes, have not received at least two prior refills of a medication indicated for type 2 diabetes, or are not fluent in English.

The primary outcome will be measured by the change in adherence measured by the validated Adherence to Refills and Medicines Scale for Diabetes (ARMS-D) survey that occurs within the study period. Survey questions will also be utilized to assess baseline demographics, type and frequency of patient reported adverse effects being experienced, along with any other relevant information. Data analysis will be done using paired t-tests. In addition to pharmacist counseling to expand patient understanding and management of medication adherence and side effects, resources from the American Diabetes Association (ADA) or other universally accepted guidelines may be provided to assist patients with controlling their diabetes.
