Dr. Jessica Wooster Thomas, Pharm.D, BCACP is a clinical pharmacy specialist focusing on Cardiology and Women’s Health at the Lt. Col. Luke Weathers, Jr. VA Medical Center in Memphis, TN. She received her Doctor of Pharmacy from UTHSC- Memphis, completed a PGY-1 residency in Phoenix, AZ with Albertsons Companies and Midwestern University-Glendale, and was a Clinical Associate Professor at University of Texas- Tyler College of Pharmacy for six years. Her clinical practice experiences include community pharmacy, ambulatory care, cardiology and transitions of care services in CHF, COPD, pneumonia, and ACS populations. Dr. Wooster Thomas has research experience and publications in her respecitive practice areas and most notably in transitions of care including the ACCP White Paper: Quality measures of clinical pharmacy services during transitions of care , Transitions of Care in Pharmacy Casebook (McGraw Hill), TOC certificate program (TSHP), amongst other practice resources developed through APhA Transitions of Care SIG. She has been an active member of the Transitions of Care SIG over 7 years and served in various roles including the SIG Coordinator, co-chair of the Practice, Communication and Education committees. Additionally, Dr. Wooster Thomas has presented APhA webinars in the past including "Transitions of Care in Pharmacy Education" focused on didactive and experiental education for student pharmacists, and a webinar titled "Transitions of Care Practice in the Outpatient Setting" focused on community pharmacy and ambulatory care pharmacy practices. She has spoken at past APhA annual meetings in San Antonio, TX presenting a CPE titled "Transitions of Care: Journey of a Heart Failure Patient Through the Continuum of Care" and in Phoeni, AZ presenting on "Transitions of Care Billing". She has recorded podcasts with iForumRx in various ambulatory care topics to provide commentary on newly released journal articles.
