(1294) A Pilot Study: Creating a Community-based Pharmacy Intervention Process Which Identifies Social Determinants of Health for Adult Patients Taking Mental Health Medication

Objectives: Social determinants of health (SDOH) are non-medical factors that contribute largely to an individual's overall health status. This pilot study focuses on creating a workflow process for pharmacy-led intervention among those patients taking mental health medications to screen for these non-medical factors that may impact medication outcomes. The objectives of this study are to: (1) Identify SDOH associated with medication adherence in patients taking mental health medications; (2) Assess participants’ views and attitudes of SDOH community-based pharmacy intervention; and (3) Assess change in participants' mental health medication adherence and any other social/health indicators following community-based pharmacy intervention.

Methods: This will be a prospective cohort study to be conducted at one regional division of a large community pharmacy chain. Recruitment and enrollment goals will include 100 eligible participants who will be identified through the use of the pharmacy dispensing system. Eligibility criteria will include participants 18 years and older currently receiving refills for their mental health medication who consent to participate in this study. Those participants who are under 18 years old or beginning new therapy for a mental health medication will be excluded. Eligible and consenting participants will be asked to complete a SDOH screening tool found within the pharmacy computer system. The survey is focused on barriers related to transportation, finances, food availability, housing, and social support. Once the screening tool is completed and evaluated, participants will be given result-based resources, including personalized handouts and pertinent information. A 2-week follow-up will be conducted, and a participant satisfaction survey will be provided to obtain feedback on the impact of the pharmacy-provided resources. The change in participant adherence to mental health medications will be calculated based on the next refill pick-up date (30- or 90-day supply based on insurance allowance). Medication adherence will be calculated using the Proportion of Days Covered method, with the goal being 80% of days covered. Statistical analysis will include descriptive statistics of demographic data, SDOH barriers, as well as views and attitudes of participants following the intervention. A password-protected computer-based spreadsheet will be utilized for data collection and analysis. All data will be reported in an aggregate fashion.
