(1295) Assess the Influence of SDOH Resource Guide on Pharmacists' SDOH Assessments

Objectives: 1. Examine the difference in the frequency of SDOH assessments conducted by pharmacists who were provided with resource guide versus pharmacists who did not receive the SDOH resource guide in high SVI stores
2. Determine the pharmacists' perception of the usefulness and relevance of the provided SDOH resource guide in their patient interactions
3. Evaluate the extent to which patients utilize social determinants of health (SDOH) referrals provided by pharmacists in community pharmacies with high SVI.

Methods: This study will use a Prospective survey design with control group. It will be carried out at pharmacies belonging to a large community chain store that have high SVI scores (10 stores). CDC created a Social Vulnerability Index (SVI), it describes patients' socioeconomic living environment and helps identify those from disadvantaged neighborhoods. It ranges from 0 (lowest vulnerability) to 1 (highest vulnerability). Pharmacies with CVI score 0f 0.75 and above will be included in the study. The staff at these pharmacies will receive training on how to use the SDOH screening tool. The SDOH screening tool consists of six questions to assess the patient’s access to transportation, ability to afford their bills, food, and housing insecurity, and support network. Patients encountered in the clinical queue (standalone queue in the pharmacy management system) for adherence and CMR interventions will be requested to complete the social determinants of health (SDOH) assessment. Patients who responded yes to any of the SDOH screening questions are included in the study. Patients who responded no to all the questions or refused to take the survey will be excluded from the study. The study will consist of two groups of pharmacists in high SVI stores randomly divided into: an intervention group receiving the resource guide. The resources can include information about local organizations, support services, and community resources that address various social determinants of health. We will compile data on 1) The number of SDOH surveys conducted by pharmacists for patients who received the SDOH pre-made resource guide and who did not receive the SDOH pre-made resource guide. 2)Surveys or questionnaires administered to pharmacists to gather their opinions on the usefulness and relevance of the provided SDOH resource guide. 3)The number of patients who accessed and utilized the referred SDOH resources. To fill their gap in care, patients who said yes to any of the SDOH questions will be referred to at least 1-2 resources. Pharmacists will help facilitate patients' access to the necessary support networks. We will then analyze the data to compare the frequency of SDOH assessment between the intervention and control groups. Apply statistical techniques to evaluate how the SDOH resource guide influences pharmacists' motivation to conduct the SDOH survey.
