(1121) Primary Care Medication Monitoring for Prep Patients

Objectives: The primary objective of this study is to evaluate guideline compliance of laboratory monitoring for patients taking HIV pre-exposure (PrEP) agents in a general internal medicine clinic. The use of PrEP has been found to be safe and effective in patients at high risk for acquiring HIV-1. However, there has been some evidence suggesting that these medications may cause significant laboratory changes. The current CDC guidelines recommend baseline and scheduled follow-up laboratory monitoring to include the following: HIV status, Hep B status, sexually transmitted infection (STI) screening, renal function, and lipid profiles. With the increased use of medication for PrEP it is important that primary care providers follow guideline recommended laboratory monitoring. Studies in this area assessing compliance with the CDC guidelines are limited but they are critical for assessing appropriate patient care and safe medication use.

Methods: This study is a retrospective, single-center, descriptive electronic medical record review for a cohort of patients in a general internal medicine clinic at an academic medical center. An electronic medical record (EMR) generated report will be used to identify patients for study inclusion. Inclusion criteria includes patients 18 years or older with emtricitabine and tenofovir disoproxil fumarate (Truvada), emtricitabine and tenofovir alafenamide (Descovy), or cabotegravir (Apretude) on their medication list and were seen in the clinic between 7/31/2020 and 7/31/2023. Data to be collected includes dates laboratory monitoring was completed in consideration of their latest clinic appointment. Medical record review of the patients will be conducted to assess laboratory monitoring compliance in reference to the suggested timing proposed by the 2021 CDC guidelines.

Results: Research is in progress.

Conclusions: We hypothesize that results will show primary care providers may not be completing appropriate monitoring for PrEP medications recommended by the 2021 CDC guidelines.
