(1023) Hypertension Action Planning Service

Objectives: In the United States, over 47% of adults have hypertension, of which 45% have uncontrolled hypertension. Uncontrolled hypertension can lead to further health complications like heart attack, stroke, chronic kidney disease, and vision loss. Pharmacists can play a huge role in helping patients manage their hypertension since they are among the most accessible healthcare professionals. Pharmacists can assess medication adherence, recommend lifestyle modifications, provide blood pressure screenings, and provide education for patients. In one study, the results show that patients who received pharmacist intervention in combination with at-home blood pressure monitoring had a greater mean absolute reduction in blood pressure compared to no intervention or home monitoring. Pharmacist intervention has been proven to show significant benefits for patients.

The main objective of this study is to see if patients who receive pharmacist intervention for blood pressure management have better blood pressure control compared to patients who do not at an outpatient pharmacy in a health system. To measure this objective, this study will evaluate the patient’s blood pressure control by looking at the patient’s blood pressure before the service and after the service and the patient’s blood pressure medications.

Methods: Patients who have hypertension and do not have an at-home blood pressure machine are identified by their provider and referred to the pharmacist for the Hypertension Action Planning Service. Patients are then scheduled for an in-person visit with the pharmacist to review the Hypertension Action Plan Booklet. Using this book, the pharmacist will review with the patient the risks of uncontrolled hypertension, their blood pressure goals, nonpharmacological interventions such as diet and exercise, medication use counseling, and proper techniques for at-home blood pressure monitoring. Patients will also create and write down SMART goals on ways to improve blood pressure control. After counseling, the pharmacist will provide the patient with a free automated blood pressure device. The intervention group will be patients who have completed the service, and the non-intervention group will be patients who have been scheduled for the service but did not complete the service. Data will be collected using the electronic medical record (EMR) and blood pressure readings will be recorded before and after the service and current blood pressure medications.
