Khyati Patel, PharmD, BCACP is an Associate Professor at the Rosalind Franklin University of Medicine and Science (RFUMS) and an Ambulatory Care Pharmacist at the Aurora Medical Center Kenosha (AMCK) Family Practice Clinic since August 2012. Khyati obtained her Doctor of Pharmacy degree from the University of Illinois at Chicago in 2011 and completed her PGY-1 Pharmacy Practice Residency at the Edward Hines, Jr. VA Hospital, Hines, IL thereafter. She collaborates with primary care physician group to provide management of various chronic diseases such as diabetes, hyperlipidemia, hypertension, osteoporosis, and obesity. She also assists patients with medication reconciliation, medication administration teaching, disease/medication education and medication monitoring for anticoagulation. In addition, she educates primary care providers and precepts introductory and advanced pharmacy practice experiential students at the clinic. In addition to providing experiential and didactic education at RFUMS, Khyati also co-produces pharmacy podcasts in the HelixTalk Podcast Series. She also is a founding faculty, faculty advisor, and faculty volunteer of the student-led Interprofessional Community Clinic at RFUMS serving underserved population of Lake County, IL. Throughout her career, Khyati has served the pharmacy profession by assuming active membership roles in American Pharmacists Association (APhA), Phi Lambda Sigma Pharmacy Leadership Society, Rho Chi Honors Society, Illinois Pharmacist Association, and Pharmacy Society of Wisconsin. She currently serves as a faculty advisor for the APhA-ASP (Academy of Student Pharmacists) and Interprofessional Community Clinic at RFUMS.
