My name is Evelyn Choe, PharmD, and I am currently serving as a PGY1 community-based pharmacy resident at University of Missouri Health Care. I obtained my Doctor of Pharmacy degree from Purdue University in 2023, where I also pursued a minor in psychology to complement my pharmaceutical studies. My professional interests lie in pioneering innovative pharmacy practices within the community setting. Specifically, I am deeply passionate about areas such as ambulatory care, specialty pharmacy, and ensuring smooth transitions of care for patients. At University of Missouri Health Care, I am actively involved in enhancing patient outcomes through various initiatives.The research project I am presenting during APhA 2024 Annual Meeting is the implementation of a pharmacist-led VTE follow-up service following ED visits. This service was designed to enhance patient care during critical transitions of care. Additionally, the project contributes to our anticoagulation ambulatory care service, further emphasizing the importance of thorough follow up in pharmaceutical care in managing complex medical conditions. I am dedicated to advancing the role of pharmacists in patient care and am committed to making meaningful contributions to the field of pharmacy.
