David Jacobs, PharmD, PhD, is an Assistant Professor in the Department of Pharmacy Practice at the University at Buffalo (UB) School of Pharmacy and Pharmaceutical Sciences. He received his Doctor of Pharmacy from the UB School of Pharmacy and Ph.D. in Epidemiology from the UB School of Public Health and Health Professions. Dr. Jacobs’ research is focused on pharmaceutical health services and implementation science with an emphasis on multi-disciplinary and collaborative research strategies. His research goal is to improve health outcomes and reduce costs for patients with multiple chronic diseases and complex medication regimens through pragmatic interventional trials, observational studies, and mixed methods. Dr. Jacobs received training in Implementation Science through an institutional K12 award (NHLBI-funded) to develop and implement a care transitions intervention for patients with COPD and other complex chronic conditions, integrating pharmacists onto the care team to perform comprehensive medication management. Other research projects include using implementation science tools to design, implement, and evaluate social determinants of health programs within community pharmacy practice and medication deprescribing initiatives across the continuum of care.
