"I’ve always had an interest in the role of the pharmacist within transitions of care and I love that my current practice site values the importance of patient-centered care. Although transitions of care continues to be a vulnerable point in the continuity of care, community pharmacists may help bridge any potential gaps between patients and providers through the convenience of telehealth services. As such, the goal of this retrospective study is to analyze the three-year impact of a community pharmacist-led transitions of care model on the various outcomes among the primary stakeholders."

I am looking forward to publishing results for other practice sites to review and potentially adopt, as well as to provide evidence to support the role of community pharmacists within transitions of care. I am also equally excited that this project provides an opportunity to evaluate the current transitions of care model implemented at my site and potentially discover ways to improve in both provider engagement and patient health outcomes."
