Innovations in Homeless Healthcare: An Interdisciplinary Wellness Approach - Part II

Individuals experiencing homelessness have limited access to medical and mental health care, prescription medications, preventive care, and other social services including case management and housing navigation. To overcome this barrier, it’s important for healthcare systems, providers and communities to implement strategies that address the unique needs of individuals without a home. The problem is the homelessness crisis, but how can pharmacists apply knowledge and skills to create a solution? Symba Center, a non-profit health and wellness organization located in California, has developed sustainable programs to address this need in this underserved population by adopting an interdisciplinary approach to addressing Social Determinants of Health. Participants of this 1-hour webinar will learn of the innovative steps Dr. Shawn R. Smith, PharmD, APh, Assistant Professor at WesternU College of Pharmacy and CEO of Symba Center, took to implement an integrative and multidisciplinary health and social service model for the homeless residents of a Southern California community.
