Improving medication safety during transitions of care (TOC) is no easy task.  Time and time again, patients with specific conditions are readmitted after transitioning out of the hospital due to preventable medication safety issues.  Pharmacists are key providers to help ensure interdisciplinary collaboration at TOC and keep patients safe.  Attend this session to put your cape on and hear best practices for optimizing safety and efficacy at TOC.
