(1290) Impact of Pharmacy-led Asthma Clinic on Asthma Control in the Primary Care Setting

Objectives: Asthma is a common health condition in adults, and it requires extensive education for patients in order to learn appropriate inhaler techniques and skills necessary for adequate self-management. There is a unique opportunity for pharmacists in the primary care setting to collaborate with physicians in the management of these patients by providing asthma-related education with the use of asthma action plans, which are recommended for every patient in the Global Initiative for Asthma 2023 Update. This study will evaluate patient improvement in asthma control after receiving education by a pharmacist.

Methods: This prospective cohort study will be conducted at a family medicine clinic in southern Illinois. A standing order will be established with physicians to establish a protocol for the pharmacist-led asthma clinic. The physicians will refer adult patients with asthma for evaluation and education by the pharmacist if they have refilled a short-acting beta-2 agonist (SABA) three times within three months or report using a SABA four or more days in a week. This will be assessed by refill data that is reported through the EHR. The patients will be evaluated for baseline asthma control. They will then receive guideline recommended treatment options based on the Global Initiative for Asthma guidelines. The pharmacist will educate patients based on a pre-established checklist containing topics such as allergen avoidance, smoking cessation, diet and exercise, medication counseling, and inhaler technique. The pharmacist will also create an asthma action plan for each patient to assist with self-management. The pharmacist will follow up with patients via a combination of in-person and telehealth visits, based on patient preference, every four weeks to assess asthma control. Once well controlled, defined by SABA usage of ≤ 2 times per week, the pharmacist will follow-up every 12 weeks.
