(1225) Impact of a Pharmacist-led Oral Chemotherapy Telehealth Support Program

Objectives: To assess the impact of a pharmacist-led telehealth support clinic on patients taking oral oncolytic medications who receive care at UnityPoint Health Allen Hospital Community Cancer Center in Waterloo, Iowa. The goal of this clinic is to aid patient understanding of oral chemotherapy treatment, alleviate adverse effects caused by oral chemotherapy, and encourage medication adherence.

Primary outcome: Impact of pharmacist-led telehealth clinic on patient outcomes
This will be measured by looking at:
- Therapy successes/continuation of therapy per protocol
- Number of patient visits to the ED/Urgent Care for chief complaints related to oral oncolytic therapy
- Resolution of adverse effects caused by oral oncolytic therapy using a Collaborative Practice Agreement to prescribe medications for symptom management
- Quality of Life (assessed using ESAS scores - Edmonton Symptom Assessment Scores)

Secondary outcomes:
- Impact on patient satisfaction with care
- Impact on healthcare cost savings (for patients and the healthcare system)

Results of this study will add to the current information available on the significance of pharmacist-led clinics for patients on oral oncolytics. Current research is focused on primarily in-person clinics and this study will help to expand the reach to the telehealth setting. Furthermore, results of this study may be used to support the implementation of similar programs and potentially additional clinical pharmacist jobs across the healthcare system.

Methods: 1. Eligible patients will be identified using reports run via Epic and based on referrals obtained from providers at the community cancer center.
2. Patients meeting inclusion criteria will be pooled and invited to participate in pharmacist-led telehealth support program and complete follow-up surveys/check-ins via MyChart or telephone related to satisfaction with care, symptoms, expectations, and medication side effects.
3. Virtual or phone call appointments with the pharmacists involved in this program will occur on a case-by-case basis based on pharmacist discretion or more frequently if requested by the patient, with newly initiated patients following up on a weekly basis and stable patients on a monthly basis.
