(1311) Improving Hypertension Disease State Management in Patients Identified as Non-adherent to ACE/ARB Therapy via Pharmacy-led Intervention

Objectives: The primary objective is to measure the change in Proportions of Days Covered (PDC) of ACE/ARB therapy. The secondary objective is the change in systolic and diastolic blood pressure from baseline.

Methods: A prospective study of hypertensive patients who have been identified has non-adherent. The study will be conducted at a large hospital in metro Atlanta, GA. Utilizing predictive analytics, the bottom 20% of non-adherent hypertension patients on Angiotensin-Converting-Enzyme (ACE) or Angiotensin Receptor Blocker (ARB) therapy, who fill their prescription with the large chain community pharmacy, will be identified. The group will be randomized into treatment and control groups via a 1:1 ratio. Patients in the control group will receive standard of care per hospital protocol. Current standard of care includes scheduling follow up appointments, appointment reminders, and answering patient questions via MyChart in EPIC. Patients in the treatment group will receive telephonic follow-up by a community-pharmacy resident at 1 month, 3 months, 6, months following the initial call. Verbal consent will be obtained during the introduction at the beginning of the initial call. The phone calls will consist of discussions based on patient-reported barriers to adherence. Discussions may also include scheduling follow up appointments, medication adherence & addressing barriers, blood pressure monitoring, verification of refills until next appointment, and offering information on digital health resources (Hello Heart for MH360) and providing free blood pressure cuffs. Patient adherence will be assessed by tracking the change in Proportion of Days Covered (PDC) scores after 6 months of phone follow-up. Patient blood pressure will be monitored at baseline and at the end of the study.
