(1289) Examining Primary Care Prescribing Patterns of Inhaled Asthma Reliever Therapies Among Adult Patients at a Federally Qualified Health Center

Objectives: Inhaled short-acting beta agonists (SABAs) are commonly prescribed rescue inhalers for immediate management of asthma symptoms or exacerbations. While SABAs play a crucial role in managing acute asthma symptoms, excessive and inappropriate use has been associated with increased risk of asthma exacerbation, hospitalization, and mortality. In 2019, the Global Initiative for Asthma (GINA) listed inhaled corticosteroid (ICS)-formoterol as the preferred asthma reliever therapy in contrast to SABAs. The purpose of this study will be to examine current asthma reliever therapy prescribing patterns at Southwest Care Center (SCC) and identify barriers to treatment in patients not on recommended anti-inflammatory reliever therapy.

Methods: A retrospective chart review will be performed using the clinic electronic medical record (EMR) system at SCC.. Adult patients, 18 years of age or older, will be included for the chart review if they had a diagnosis of asthma based on specific ICD 10 codes, had an asthma-related clinic visit, or were prescribed asthma reliever therapy between 01/01/2022-12/31/2022 at SCC. Asthma reliever therapies are inhaled SABAs, inhaled short-acting muscarinic antagonist (SAMAs), and ICS-formoterol. Patients will be excluded if they have chronic obstructive lung disease, lung disease caused by COVID-19 virus, or were not seen at SCC clinics. Full exclusion criteria are listed in the research protocol. Approximately 583 charts will be reviewed from all SCC clinic locations based off asthma-related encounter estimates using ICD 10 codes, SABA/SAMA inhaler prescriptions, and ICS-formoterol inhaler prescriptions. The primary outcome of this study is to determine the percentage of ICS-formoterol prescriptions for asthma treatment. Secondary outcomes include percentage of SABA/SAMA inhaler prescriptions for asthma treatment, identification of patients not on anti-inflammatory reliever therapy for asthma and determining barriers to treatment, and rates of glucocorticoid use in both ICS-formoterol and the SABA/SAMA group. Descriptive analysis will be done to describe the patient population. Multivariate logistic regression will be used to evaluate asthma exacerbations by assessing emergency department visits, short term steroid prescriptions, and documented exacerbations.
