(1054) Assessing Outcomes Related to Glucagon-like Peptide 1 Interclass Switch in Patients with Diabetes

Objectives: Glucagon-like peptide 1 (GLP-1) agonists are a first line treatment option for patients with type 2 diabetes, and have shown benefits in A1c lowering effects, atherosclerotic cardiovascular disease protection, and weight loss benefits. However, each medication within this class has shown differing effects on these outcomes. One study assessing outcomes in patients switching from liraglutide or dulaglutide to semaglutide showed A1c lowering effects of 0.65% and weight loss effects of 1.69 kg. However, there is still limited data surrounding switching from semaglutide to another GLP 1. The purpose of this study is to determine whether or not interclass GLP-1 switching due to a change in cash price and prior authorization requirements for insurance companies negatively impacts patients’ health. The primary objective of this study is to retrospectively assess the effect on A1c after switching from a once weekly semaglutide to any other injectable GLP-1 therapy. The secondary objective of this study is to retrospectively evaluate the effect of both weight and adherence upon switching from a once weekly semaglutide to any other GLP-1 therapy.

Methods: A retrospective chart review will be completed for any patients with type 2 diabetes who were taking stable doses of injectable semaglutide and switched to another GLP-1 injectable medication. Patients included in the study must be under the care of a Five Rivers Health Centers physician, have a diagnosis of diabetes, have been taking injectable semaglutide for at least 4 weeks before switching to another GLP-1 agonist, and have a minimum of 1 follow up visit with HBA1C after the medication switch. Baseline A1c, weight, and medication adherence will be obtained for each patient before the GLP-1 switch. Medication adherence will be measured by the percent of days covered (PDC). Follow up A1c, weight, and medication adherence will also be measured after the GLP-1 switch. Baseline data will be compared to the most recent data point available. The primary and secondary objectives will be evaluated using a paired t-test.
