(1313) Resolving Adherence Barriers with Adalimumab in Community-based Specialty Pharmacies

Objectives: Medication adherence is an important aspect of patient care; a lack of which can become a barrier to treatment. Community-based specialty pharmacies are well-equipped to closely follow-up with patients’ needs, specifically those taking high-risk and/or limited distribution drugs. This study will assess the outcomes of patients willing to change dispensing channels to receive additional clinical adherence services and monitor the impact of pharmacists’ clinical interventions on adherence. In addition, the top adherence barriers in this patient population will be identified.

Methods: This intervention will take place across a 90-day span (July through October 2023) across a total of 19 participating pharmacies in Tennessee and Michigan. 459 candidate patients taking adalimumab with a proportion of days covered of less than 80% have been identified; at the midpoint of the pilot, 53 patients agreed to transfer their medication and are participating in the pilot. The proportion of days covered from three months prior to the intervention and at three and six months after the intervention will be measured. Analysis of patients’ data from specialty pharmacies will involve retroactive chart review within the pharmacies’ online clinical management platforms, as well as tracking their adherence by measure of their fill-date and pickup-date frequency before and after the intervention. From the community-based specialty pharmacists’ patient care plan notes that have been recorded, barriers to patient medication adherence will be categorized.
