(1068) Identifying Social Determinants of Health During Pharmacy-led Blood Glucose Screening and Testing for T2DM or Pre-diabetes at Community Events

Objectives: The objective of the study is to identify social determinants of health affecting pre-diabetic and diabetic patients through pharmacy-led screening, point of care (POC) testing, and follow up interventions on people testing abnormally.

Methods: The study data collection will occur at different community health events held in Broward or Palm Beach Counties and the attendees of the events will be the targeted subjects. Participants who come to the blood glucose (BG) screening table will be informed of the study objectives and given the option to participate in the study. Participants who wish to participate will sign the written/electronic consent form and fill out a 20-question Microsoft Forms survey which includes various questions on demographics that affect social determinants of health and pre-diabetes/diabetes screening questions (adapted questions from ADA’s pre-diabetes screening tool). Participants will then receive POC BG testing performed by student pharmacists and the results will be inputted into the last question of the survey.

Results: Research and data collection is still in progress and data collection will continue at future community events. At the first community event, ADA’s Step out to Walk to Stop Diabetes, 64 participants consented to participate in the study. In the preliminary results, the majority of the participants were females (73%), Hispanic/Latino (42%) or Black, non-Hispanic (36%), and an average age of 42 years old. The majority had a bachelor's degree (40.6%) with an annual household income level of $120,000 (26.6%). Out of those individuals, 14% were previously diagnosed with T2DM, 9% had prediabetes, and 11% had abnormally elevated BG levels, with an average BG level of 245 mg/dL. Six participants were considered at risk of T2DM using the ADA screening tool, yet had not been diagnosed with pre-diabetes. Four participants needed to be referred to local Urgent care for elevated BG levels and will require further follow-up.

Conclusions: Research and data collection is still in progress and data collection will continue at future community events before a conclusion is drawn.
