(1282) The Role of Ambulatory Care Pharmacist in Meeting "Healthy People 2030" Objectives

Objectives: 1. Describe overaching goals of the "Healthy People 2030" initiative
2. Examine the role of pharmacist in Remote Patient Monitoring (RPM) to enhance paient economic, clinical and humanistic outcomes (ECHO)
3. Provide recommendations on enhancing patient care to improve public health

Methods: This study was conducted using a systematic review tool (Covidence). The researchers used the following databases to answer the research question; databases including PubMed, Google Scholar. The inclusion criteria included US studies the following: published from February, 2023 to March, 2023; search terms included Remote Patient Monitoring (RPM), diabetes, hypertension, obesity; conducted in the US. Social Determinants of Health (SDOH) were used according to “Healthy People 2030”. SDOH factors were defined based on the following five factors; 1. Economic Stability, 2. Education Access and Quality, 3. Health Care Access and Quality, 4. Neighborhood and Built Environment and 5. Social and Community Context. To reduce bias, at least two investigators independently evaluated each article. Moreover, to increase internal and external validity of the study, two additional senior researchers reviewed the articles with identified discrepancies and resolved the conflict.

Results: This research study identified a total of 300 articles.100 articles met the inclusion criteria. Based on preliminary data analysis, the majority of the studies there were improvements in patient outcomes clinically and humanistically. One study that was conducted in patients with type 2 diabetes and obesity with body mass index >=25, showed that participation alone resulted in improved blood glucose control (mean HbA1c declined: 8.5% to 7.5%, p = 0 01). Mean weight loss and additional blood glucose decreases. In another study in patients with hypertension for 3 months showed an average blood pressure decrease in the virtual collaborative care clinic (vCCC) arm. Patients were likely to have BP < 140/90 mm Hg (55%) compared to patients in the group with home blood pressure monitors only (37%) (p = 0.001). An additional study was conducted to assess two interventions. First intervention being lifestyle coaching, and second intervention being remote support. Results of the study revealed that both interventions achieved greater net improvements in waist circumference and fasting plasma glucose. Additional studies found greater improvements in the following outcomes: activation, self-care, and Quality of Life (QOL), increase in medication regimen intensity; compared with the patients that did not participate in RPM.

Conclusions: Healthy People 2030 initiatives include Leading Health Indicators, a subset of high-priority objectives that encompasses the patient's life span. The prevalance of chronic diseases such as hypertension, diabetes, obesity. This research study highlights the vital role of ambulatory care pharmacists in monitoring atient outcomes remotely. Based on the finding of this research, remote patient monitoring not improved clinical and humanistic outcomes. Furthermore RPM provided an effective, innovative, and affordable technology to better manage chronic diseases to improve public health in ambulatory care practice settings.
