(1123) Vaccine Adherence in People Living with HIV Utilizing Specialty Pharmacy Services in an Outpatient Clinic Setting

Objectives: Vaccine adherence is a pivotal factor in the health outcomes of individuals with Human Immunodeficiency Virus (HIV) due to their potentially compromised immune systems. An urban outpatient clinic setting, offering specialty pharmacy services, collects vaccine adherence information in people living with HIV who utilize these services. The purpose of this study was to determine the adherence rate of each recommended disease-specific vaccine among people living with HIV who utilized the specialty pharmacy services, and determine the overall adherence rate for the clinic.

Methods: The electronic medical record and the state vaccine adherence registry were quantitatively reviewed to determine vaccine adherence of patients who had a clinic visit and encounter with the specialty pharmacy services between August 1, 2022 and July 31, 2023. Patients were included if they were actively utilizing the specialty pharmacy services between the specified timeframe, 18 years or older, diagnosed with HIV, and an Arkansas resident. They were excluded if they had a documented allergy to a vaccine or were uncontrolled on their HIV treatment regimen. Patients not controlled on their regimen (CD4 ≤ 200 cells/ mm3) were excluded based on the potential to not receive a vaccine due to lack of immune response leading to delayed vaccine administration. Patient records were randomly selected to include 25% of qualifying charts. Data collected included patient demographics, HIV infection (CD4 count and HIV RNA), and vaccine adherence at the point of their last visit.

The 2023 CDC guideline-recommended vaccines will be assessed for adherence at the time of the participant’s last visit to the HIV clinic, which includes: zoster; human papillomavirus; pneumococcal; hepatitis A; hepatitis B; and meningococcal ACWY. Each patient will be assessed for eligibility to the recommended vaccines. Patients will be classified as adherent if they were eligible and received the vaccine or started the series. Patients will be considered nonadherent if they were 1 year or more behind on each recommended vaccine. For age-based vaccines, patients will be included if they met the age criteria for each specific vaccine. Quantitative analysis will be evaluated through descriptive statistics. The primary endpoint is the total composite adherence rate across all vaccines which will be calculated and compared to the clinic’s goal of 80% adherence. A secondary endpoint of adherence rates for each vaccine across all included patients will also be calculated.
