(1002) Assessing the Impact of Community-based Specialty Pharmacist Provided Blister Packaging Intervention in Neurology Patients Taking Three of More Oral Medications

Objectives: To assess the impact of community-based specialty pharmacists provided blister packing intervention on adherence at baseline and post blister-packing utilizing portion of days covered (PDC) calculation. To assess patient satisfaction via satisfaction survey.

Methods: The study will be a prospective, analytic, single-centered center. We have 32 patients that are eligible for enrollment. They must be 18 years old, diagnosed with multiple sclerosis, epilepsy, tardive dyskinesia, Parkinson’s disease, or Alzheimer’s disease on 3 or more oral maintenance medications, and are currently non adherent via PDC calculation. PDC of less than 80% over the past 6 months will be deemed as non-adherent. Patients must also live in 1 of the 10 stated that are eligible to be shipped to from the pharmacy in TN. Patients will not be eligible if they are already receiving blister packaged medications, pregnant, non-English speaking, or taking medications that must be dispensed in their original container (Epidiolex (cannabidiol), Inbrija (levodopa), Tecfidera (dimethyl fumarate). Patients will be identified by using the pharmacy’s care management, Mediation Therapy Management (MTM), and specialty pharmacy platforms and prescription processing system. The care management platform flags patients that are late for pick up or overdue for refills. The MTM platform will be utilized to identify patients that are non-adherent by using their PDC calculation and notification of Targeted Intervention Programs (TIPs) and Comprehensive Medication Reviews (CMRs). The clinical monitoring program which shows how many times the patient has been contacted for a refill and prescription processing system will be utilized for refill history information, specifically sell dates of prescriptions. Once patients have been identified via the pharmacy’s patient care management system, the patients will be transferred via the telephone to the resident to inform them about the study. If the resident is not present, the patient will be directed to a neurology pharmacist for enrollment. The pharmacy resident will obtain verbal consent from the patient to enroll them into the study. At the time of consent, an intake form will be completed to collect demographic information, missed doses, medication history, health condition history, and reasons for non-adherence. The pharmacy resident will then calculate a baseline PDC and formulate an appropriate blister pack medication list with the patient and providers if necessary. PDC will only be calculated on medications included in the blister pack. The medications will be synchronized to be refilled at the same time. Patients will receive a blister pack each month for up to six months. A follow-up will be conducted by the resident after each 30-day period for a maximum of 6 months. During the follow-up, a patient satisfaction survey will be conducted and PDC will be calculated and documented.
